I've not been very well recently, I faint a lot && currently have a heart monitor on for 7 days.

Oh I’m terribly sorry to hear that. I hope you feel well soon :) you should definitely tell him. Just go up to him start a nice conversation then say “I want to tell you something” then when he asks what it is just say I like you :) hope that works out, feel better!
